What the FREC 4 course covers
Advanced airway and respiratory care
Supraglottic airway devices, advanced oxygen and ventilation strategies, respiratory emergency management and escalation pathways.
Trauma and major incident care
Advanced haemorrhage control, chest decompression principles, splinting and immobilisation, triage and working within a multi-agency major incident response.
Medical emergencies and medicines
Cardiac, neurological, endocrine and obstetric emergencies, sepsis and anaphylaxis, plus the wider FREC 4 medicines range and safe administration.
Assessment and professional practice
Scenario-based practical assessments, written examination, clinical governance, documentation, safeguarding and reflective practice.
Entry requirements and assessment
You must hold a valid Level 3 First Response Emergency Care qualification, or an accepted equivalent, before starting FREC 4. Assessment combines continuous practical observation, scenario-based assessments and a written examination. Successful candidates receive the Qualsafe Awards Level 4 Certificate in First Response Emergency Care (RQF), valid for three years.
Your five days on FREC 4
Day 1 — Enhanced assessment and clinical governance
Scope of practice, clinical governance and consent, structured history taking, detailed physical examination and vital sign interpretation, documentation and handover.
Day 2 — Airway, breathing and monitoring
Supraglottic airway devices, advanced oxygen therapy and assisted ventilation, capnography, 3-lead monitoring and respiratory emergency management.
Day 3 — Trauma and major incident
Advanced haemorrhage control, chest injury management, splinting and immobilisation, entrapment and prolonged care, triage and multi-agency response.
Day 4 — Medical emergencies and medicines
Cardiac, neurological, endocrine and obstetric emergencies, sepsis and anaphylaxis, plus the FREC 4 medicines range and safe administration under governance.
Day 5 — Assessment
Scenario-based practical assessments, skills sign-off, written examination and reflective practice against Qualsafe Level 4 criteria.

What is a FREC 4 course?
FREC 4 is the Qualsafe Awards Level 4 Certificate in First Response Emergency Care. It extends the FREC 3 scope of practice with enhanced patient assessment, additional clinical skills and a wider range of medicines, for responders working in event medicine, industry, offshore and pre-hospital care.
Do I need FREC 3 before FREC 4?
Yes. A valid Level 3 First Response Emergency Care qualification (or an accepted equivalent) is a prerequisite for enrolling on FREC 4.
How long is the FREC 4 course?
The CM Marine FREC 4 course runs over 5 days of intensive classroom, practical and scenario-based training, with formal assessment built into the programme.
How long is FREC 4 valid for?
FREC 4 is valid for 3 years. Ongoing clinical practice and annual refresher training are strongly recommended, and you must requalify before the certificate expires.
What roles does FREC 4 support?
FREC 4 supports enhanced event medical roles, ambulance and patient transport work, offshore and marine medic support, and industrial or remote site medical cover where a higher level of care is required.
