FREC4 (First Response Emergency Care Level 4)
Advanced Qualsafe Level 4 pre-hospital care qualification for FREC 3 holders moving into enhanced clinical roles offshore, at sea and at events.
About this course
FREC 4 builds on FREC 3 with enhanced patient assessment, supraglottic airway management, ECG acquisition and the administration of specified medicines under Patient Group Directions. It is the recognised qualification for enhanced pre-hospital care providers working under clinical governance, and a common stepping stone to associate ambulance practitioner roles. The course combines pre-course study, classroom delivery, extended scenario work and a portfolio of evidence. Valid for three years.

Course syllabus
Enhanced patient assessment
- Detailed history taking, SOCRATES and secondary survey
- Observations, NEWS2 scoring and clinical interpretation
- Anatomy and physiology refresher across all body systems
Advanced airway and respiratory care
- Supraglottic airway insertion and management
- Advanced oxygen therapy and assisted ventilation
- Respiratory conditions and nebulised medication
Circulation and cardiac care
- 3-lead and 12-lead ECG acquisition and basic interpretation
- Advanced haemorrhage control and fluid awareness
- Cardiac arrest management within a team
Medicines and clinical governance
- Administration of specified medicines under Patient Group Directions
- Legal, ethical and safeguarding responsibilities
- Clinical record keeping and information governance
Complex trauma and specialist patients
- Major trauma, entrapment and prolonged care
- Paediatric, maternity and older patient considerations
- Mental health presentations and de-escalation
What you will be able to do
- Work as part of an enhanced pre-hospital care team under clinical governance
- Carry out advanced assessment and monitoring, including ECG acquisition
- Administer specified medicines safely within a governance framework
- Manage complex trauma and medical presentations in prolonged care settings
Entry requirements, assessment and certification
- Entry requirements
- Learners must hold a current FREC 3 (or equivalent Level 3 pre-hospital qualification) and be at least 18 years old. Pre-course reading is issued on booking.
- Assessment
- Portfolio of evidence, practical scenario assessments, skills sign-off and written theory papers assessed against Qualsafe Level 4 criteria.
- Certification
- Successful learners receive the Qualsafe Level 4 First Response Emergency Care (RQF) qualification, valid for three years.
